Preliminary Adequacy Issues for CN-01 17 <br />May Day Idaho Mine Complex <br />22. The illegal Chief Portal was constructed without the Division's review and approval. The <br />excavation undermined the access road to the Incas Mine, creating a near vertical <br />highwall in unconsolidated soils which terminates at the edge of the road. Excavated <br />materials were inappropriately placed across Little Deadwood Gulch, creating an <br />embankment across the channel and obstructing flow. <br />(a) on page four of the Geotechnical Stability Exhibit the application proposes to <br />address these issues with a channel design, provided in Table 6.5-1. Table 6.5-1 was <br />not included with the application. Please provide Table 6.5-1. <br />(b) Please provide a work plan for reclamation of the affected portion of Little <br />Deadwood Gulch, with topographic map(s) of appropriate scale illustrating the work <br />plan and the final configuration of the reclaimed drainage channel. <br />(c) Please provide a geotechnical evaluation for the portion of the access road to the <br />Incas Mine, undermined by construction of the Chief Portal. The geotechnical <br />evaluation shall address the applicable requirements of Rules 6.5(1), (2) and (3). <br />23. Attachment 6.5.1 suggests steel sets will be constructed at the Lamb Portal. Please <br />clarify whether the Lamb Portal will be re-opened and stabilized with steel sets. <br />24. Please submit road designs, prepared and certified by a professional engineer, for the <br />proposed access roads to the eleven drill pads. <br />Exhibit E - Reclamation Plan, Rule 6.4.5 <br />25. The application indicates the drill pad access roads are permanent and will not be <br />removed during final reclamation. The Act and Rules require affected lands to be <br />reclaimed to a stable configuration and beneficial use. Please demonstrate how the <br />access roads to the eleven drill pads will be stabilized. Please discuss how the proposed <br />roads will benefit the post-mining land use. Please provide maps with the reclamation <br />plan illustrating the final configuration of the drill pads and their associated access <br />roads. <br />26. The application indicates the illegal access road, from County Road 124 to the bridge, <br />will be removed during final reclamation. Please provide a map with the reclamation <br />plan illustrating the final configuration of the reclaimed access road corridor. <br />27. The application states the mill facility and steel sets at May Day 1 level will be <br />dismantled to the extent possible. Please revise the reclamation plan to indicate total <br />dismantlement of the mill facility, steel sets and rubblization of the concrete floor and <br />foundation for the mill facility. The Division cannot approve burial of intact structures as <br />permanent reclamation. <br />28. The reclamation plan proposes to permanently dispose mill tailings and debris from the <br />dismantled mill facility in the underground workings. The application does not
