PiAeda <br />Director <br />The Division of Reclamation, Mining, & Safety (Division) has reviewed the materials that Mr. Ewegen <br />provided by fax in response to the Division's Incompleteness Notice dated 31 January 2011. Our <br />review indicates that the application is still incomplete. The additional information described below <br />must be provided before the Amendment application AM-05 may be designated as complete and filed. <br />1) Exhibit C The Pre-mining and Mining Plan Map(s) of Affected Lands do not meet the <br />standards specified in Rule 6.2.1 with respect to quality and preparation by <br />qualified persons. Please re-submit revised maps that conform to the Rule and are <br />also of sufficient size (8 x 11 at a minimum) to adequately display the detail <br />needed for Division review. <br />2) Exhibit E Reclamation Plan. Please describe all planned reclamation activities including <br />equipment removal, demolition and disposal, haulage, grading, ripping, tilling, <br />etc., in addition to the revegetation plan already provided. <br />3) Exhibit F Reclamation Plan Map does not meet the standards specified in Rule 6.2.1 with <br />respect to quality and preparation by qualified persons. Please re-submit a revised <br />map that conforms to the Rule and is also of sufficient size (8 x 11 at a <br />minimum) to adequately display the detail needed for Division review. <br />4) Exhibit L Please provide an updated reclamation cost estimate including all items listed <br />above regarding Exhibit E. <br />5) Exhibit S Permanent Man-made Structures. The applicant still has not provided a list of <br />permanent man-made structures around the new area to be added as part of this <br />Amendment AM-4, nor plotted them in readable fashion on Exhibit C map, along <br />Office of Office of <br />Ninon I anti Rprlamatinn Denver • Grand lunction • Durango Active and Inactive Mines
