PERMIT #: M-2000-116 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: January 26, 2011 <br />OBSERVATIONS <br />This was a routine inspection performed by the Division as part of its monitoring of Construction Materials <br />112c permits. The operator was contacted about the scheduled inspection and was present throughout the <br />inspection. The site was not active at the time of the inspection. There was no equipment in the permit area. <br />Availability Of Records: <br />The operator is current with annual fees and annual reports. There has been no excavation, aggregate <br />processing or significant hauling of product to report in several years, but there has still been activity at the <br />site. The operator was reminded that all site activities should be reported, including backfilling and grading, <br />sales of smaller amounts of material, reclamation, seeding and weed control. <br />Backfilling and Grading: <br />There has been a large amount of backfilling of fines on the north side of the pit, in conformance with the <br />approved permit plans. <br />Financial Warranty: <br />The bond amount is $80,423 in the form of a letter of credit. The reclamation costs have not been reviewed in <br />several years, and the Division must review them again. The updated figures will be sent to the operator <br />under separate cover. The operator is reminded that if the new figures indicate an increase in bond amount, <br />the operator has up to 60 days to provide the additional bond. Until the new figures are generated, the <br />current bond amount is considered adequate. <br />Processing Waste: <br />There are two stockpiles of waste asphalt on the site, one in the pit and one in the stockpile area in the NE <br />portion of the site. The pile in the pit is gradually being removed and recycled or used offsite. Once it is gone <br />further pit backfilling with fines may occur. <br />Roads: <br />The roads are well-constructed, extending along the northern boundary and southward through the center of <br />the site to the pit. The roads will be reclaimed at the conclusion of mining activity. <br />Revegetation: <br />No seeding for final reclamation has been done, but the topsoil stockpiles are seeded to temporary cover. The <br />operator is monitoring for noxious weeds, and will treat them as needed. <br />Signs and Markers: <br />The required permit ID sign is posted on the entrance gate, and the permit boundary is the fenceline <br />surrounding the site. All activities are within the boundary. <br />Topsoil: <br />The topsoil for replacing on the pit is stockpiled around the W, S, and E sides of the pit. The topsoil for <br />replacing on the access and haul road is windrowed along the road. These stockpile locations will facilitate <br />easy replacement. The topsoil is free of noxious weeds and does not exhibit erosion. As new areas are <br />disturbed in future mining or processing activity, the operator is reminded to continue carefully stripping <br />Page 2 of 5
