AGENCY: <br />NA None None <br />WEATHER: INSP R' SIGNAT SIGNATURE DATE: <br />Cloudy February 3, 2011 <br />L <br />The following inspection topics were identified as having Problems or Possible Violations OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Acid And Toxic Materials <br />PROBLEM/POSSIBLE VIOLATION: Problem: There is a debris pile in the NE corner of the permitted area, that <br />includes a 55-gallon drum of what appears to be waste oil. The drum is on its side and leaking, contaminating <br />the soil and other underlying materials. <br />CORRECTIVE ACTIONS: The operator must promptly remove this leaking drum and any other vessel containing <br />waste materials or potential contaminants, as well as all contaminated earthen materials, and properly dispose <br />of all of these items offsite. Evidence of completing these actions (in the form of photos and description) must <br />be provided to this office by the correction date shown herein. <br />CORRECTIVE ACTION DUE DATE: 3/04/11 <br />Page 1 of 5
