6.3.5 Exhibit E -map <br />15. Please include the following information on the Mining Plan Map as required <br />by Rule 6.3.5(2) (b): <br />i. Label the names of owners of record of the surface of the affected area and <br />of the land within two hundred (200) feet of the affected area. <br />The owners of record and all adjacent owners have been affixed to the Mining <br />Plan Map. The revised map is attached <br />ii. Identify the owner of the substance to be mined. <br />The owner of the substance to be mined has also been affixed to the Mining Plan <br />Map. <br />iii. Identify the type of structure and owners of record of any permanent or <br />man-made structures within two hundred (200) feet of the affected area. <br />There are no permanent or man-made structures within two hundred feet of the <br />affected area. This has been noted on the Mining Plan Map. <br />16. The Reclamation Plan states the access driveway will remain as part of final <br />reclamation. Please revise the Reclamation Plan Map to show the location and <br />approximate dimension of the access driveway. <br />The location and approximate dimension of the access driveway is now noted on the <br />Reclamation Plan Map. A revised copy is attached. <br />Additional Information <br />Two changes from the original application are herein presented: <br />1) Washington County, after additional consideration, plans to slide the pit site <br />westward about 175 feet to avoid the pre-existing disturbance. I feel this is a minor <br />change in the application. It will not affect any new adjacent landowners. In fact, the <br />adjacent landowner to the east should have any previous impact minimized because of <br />the move. All the land use in that area, for many miles, is primarily rangeland. <br />5 <br />
