iv. Specify the application method for grass and forb seeding. If the seed is to be <br />broadcast, the application rate shall be twice the rate required for seed drilling. If the <br />seedbed has not been adequately roughened prior to seeding, the seed shall be raked <br />or harrowed after broadcast application. <br />V. If mulch is needed, specify the kind to be used, the crimping method, and rate of <br />application. <br />13. The Reclamation Plan Map shows the disturbance in the southeast corner of the pit. Please <br />clarify if the Operator will reclaim the pre-existing disturbance. The Operator should note <br />that if any new disturbance is created in this area, then the Operator will be responsible for <br />reclaiming the area. <br />14. The Final Contour Plan shows the east pit wall will be graded to 3H:1 V from the edge of <br />County Road XX. Will the Operator construct any stormwater controls on the east pit wall? <br />If not, please describe how stormwater runoff will be prevented from the entering pit from <br />County Road XX. <br />6.3.5 Exhibit E - Map <br />15. Please include the following information on the Mining Plan Map as required by Rule <br />6.3.5(2)(b): <br />i. Label the names of owners of record of the surface of the affected area and of the <br />land within two hundred (200) feet of the affected area. <br />ii. Identify the owner of the substance to be mined. <br />iii. Identify the type of structure and owners of record of any permanent or man-made <br />structures within two hundred (200) feet of the affected area. <br />16. The Reclamation Plan states the access driveway will remain as part of final reclamation. <br />Please revise the Reclamation Plan Map to show the location and approximate dimensions of <br />the access driveway. <br />6.3.6 Exhibit F - List of Other Permits and Licenses Required <br />No comment. <br />6.3.7 Exhibit G - Source of Legal Right-to-Enter <br />No comment. <br />6.3.8 Exhibit H - Municipalities Within a Two-mile Radius <br />No comment.
