4. The latitude and longitude coordinates provided for the primary mine entrance in the Permit <br />Application do not match the latitude and longitude coordinates listed on the Location <br />Map(s). Please clarify which coordinates apply to the primary mine entrance. <br />6.3.2 Exhibit B - Site Description <br />5. The Applicant has stated the vegetation at the site consists of native grasses. Please specify <br />the principal species of native grasses present at the site. <br />6. The Applicant has stated the water table is approximately 20 - 25 feet deep. Please inform <br />the Division how the depth of the water table was determined. <br />6.3.3 Exhibit C - Mining Plan <br />7. The Mining Plan states no slopes will be left greater than 3H:1V. Will the pit walls be mined <br />at 3H:1 V or reclaimed to 3H:1 V? If the pit walls will be reclaimed to 3H:1 V, then specify the <br />grade of pit walls during mining. <br />8. The Mining Plan states there is an old area of disturbance at the southeast corner of the site; <br />the Location Map denotes a gravel pit in this location. Please describe the nature of this <br />disturbance, including approximate dimensions of any pit walls as required by Rule 6.3.3(f). <br />Also, please indicate if the disturbance is the result of mining. <br />9. The Mining Plan states excavation will stop at least one foot above the static groundwater <br />level. At least two feet of native material should be left over the groundwater table. Please <br />commit excavating no closer than two feet above the static groundwater level. <br />10. The Mining Plan states limited processing may occur. Please describe the nature of <br />processing as required by Rule 6.3.3(m). <br />11. The Mining Plan Map shows the top of the east pit wall is immediately adjacent to County <br />Road XX. Will the Operator maintain a setback from County Road XX? <br />6.3.4 Exhibit D - Reclamation Plan <br />12. The Applicant has stated the site will be seeded according to NRCS recommendations. The <br />Division generally concurs with the seeding recommendations of NRCS; however, the permit <br />application did not contain their recommendations. Please provide the Division with the <br />following information as required by Rule 6.3.4(1)(c)(ii)-(v): <br />ii. Explain how the seedbed will be prepared to eliminate compacted conditions. State <br />the type, application rate, and soil incorporation methods of fertilizer application, if <br />any. <br />iii. State the grass, forb, shrub and tree species to be planted and the applicable <br />quantities. Specify the quantity of each grass species and forb species as pounds of <br />pure live seed per acre.
