An AMEC field representative was present during placement. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC inspected and approved subgrade prior to SLF placement. <br />AMEC observed SLF placement. <br />AMEC inspected and approved subgrade prior to geomembrane deployment. <br />AMEC observed, logged data, and assisted in the installation of geomembrane within the Phase 5 VLF. <br />AMEC inspected and approved primary geomembrane for further construction activities. <br />AMEC observed and inspected anchor trench excavation/backfill. <br />AMEC observed the installation of the leak detection system. <br />AMEC observed, obtained samples, and assisted in the process of DCF material at the DCF process area. <br />AMEC continuously monitored the DCF placement. <br />AMEC observed the installation of the high volume solution collection system. <br />AMEC observed loading ore at the Phase 5 PSSA during day and night shift. <br />AMEC observed construction and placement of the level indicator piping. <br />VLF Weekly Report WE 11.
