<br />• Observation of Phase 5 PSSA/Phase IVa and Phase IVb tie-in.
<br />Prior to the deployment of geomembrane, AMEC and Lone Star verified that the subgrade
<br />was free of soft spots, protrusions, angular particles, abrupt grade changes, and loose soil.
<br />AMEC and Lone Star verified that the SLF, LVSCF, and ore slope surfaces had been
<br />proof-rolled, and that the SLF surface was free of desiccation cracks. Where the surface of
<br />the SLF, LVSCF, or ore slope did not meet specifications, Ames repaired the area and
<br />re-compacted the surface. Lone Star provided written certification of geomembrane
<br />subgrade acceptance. The subgrade acceptance certificates are presented in Appendix G-
<br />2 and Appendix G-3 for the Phase 5 PSSA secondary and primary Geomembrane layers,
<br />respectively.
<br />AMEC assigned each geomembrane panel a number, as follows:
<br />• Single-sided textured 100-mil LLDPE geomembrane panels deployed in
<br />the PSSA were designated by S-1 through S-87, S-96 through S-287,
<br />S-330 through S-366, S-419 through S-422, S-455 through S-463, S-480
<br />through S-497, S-525 through S-861, P-319 through P-337, P-377
<br />through P-392, P-461 through P-477, P-484 through P-485, P-496
<br />through P-497, P-563, P-570, P-573, P-585, and P-637 through P-640;
<br />• Double-sided textured 100-mil LLDPE geomembrane panels deployed in
<br />the PSSA were designated by S-464 through S-479, S-498 through
<br />S-524, P-314 through P-318, and P-339; and
<br />• Smooth 100-mil LLDPE geomembrane panels deployed in the PSSA
<br />were designated by S-88 through S-95, S-288 through S-329, S-367
<br />through S-418, and S-423 through S-454, P-1 through P-313, P-338,
<br />P-340 through P-376, P-393 through P-460, P-478 through P-483, P-486,
<br />P-489 through P-495, P-498 through P-562, P-564 through P-569, P-571
