PERMIT #: M-1981-277 <br />INSPECTOR'S INITIALS: MAC <br />WQPP('TT()N DATE' Nnvamhar 16 ?010 <br />appropriate Technical Revision fee of $216.00 by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 12/24/10 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety (Division). <br />Mike Loukonen of Loukonen Bros. was also present for the inspection. The Beech Hill Quarries site is located <br />approximately 1 mile north of Lyons, Colorado. The site is permitted for 190 acres with a maximum allowed disturbance <br />of 31 acres. The post-mining land use is rangeland. <br />Acid And Toxic Materials: <br />The mining equipment is fueled onsite from a tank located in the Kupio Quarry. The secondary containment structure <br />around the tank is damaged and will not function properly in the event of a fuel spill. The Operator stated the tank did not <br />contain fuel; there was no evidence the fuel tank was leaking at the time of the inspection. The damaged secondary <br />containment structure is considered a problem and will require corrective action by the Operator. <br />Financial Warranty: <br />The Division currently holds a bond in the amount of $59,500.00. The Division is required to hold a financial warranty in <br />a sufficient amount to complete reclamation of the site pursuant to C.R.S. 34-32.5-117(4)(b)(I). Based on observations <br />made during the inspection, the current financial warranty is sufficient to complete reclamation. <br />Hydrologic Balance: <br />The Helsinki Quarry and the Gust Quarry are both impounding surface water runoff. Hydrophytic vegetation was <br />observed around the Helsinki Quarry impoundment, indicating the water had been present longer than 72 hours. The <br />Operator should note that all surface water must be released to the natural drainage system within 72 hours, impounding <br />water longer than 72 hours will require the Operator to replace evaporative depletions through the Office of the State <br />Engineer. The Operator stated a Stormwater Discharge Permit has been issued to the mine operation, allowing pumping of <br />the water from the impounments. The Division does not have a copy of the Stormwater Discharge Permit on file; this is <br />considered a problem and will require corrective action by the Operator. <br />Gen. Compliance With Mine Plan: <br />The Beech Hill Quarries site is comprised of 7 individual quarry areas and a staging area. The status of each of the <br />quarries is outlined below: <br />Helsinki: <br />This quarry has not been mined for the past 3 years; no reclamation has taken place. Approximately 1.5 acres have been <br />disturbed. <br />Gust: <br />This quarry is currently being mined. The site is adjacent to the Barnard Quarry (M-1996-079). Approximately 1.2 acres <br />have been disturbed. <br />Soumi: <br />No active mining is taking place in this quarry. Overburden has been separated and piled around the perimeter and will be <br />used to recontour the site during final reclamation. Approximately 5.1 acres have been disturbed. <br />Kupio: <br />No active mining is taking place in this quarry. Approximately 6.8 acres have been disturbed. <br />Emil: <br />This quarry was being mined during the inspection. Most of the saleable product is immediately taken down to the yard <br />for sale. Approximately 4.6 acres have been disturbed. <br />Dump: <br />This site is adjacent to the Emil Quarry. The dump consists of an overburden pile which has been contoured to match the <br />Page 2 of 5
