OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Backfilling & Grading <br />PROBLEM/POSSIBLE VIOLATION: Problem: Trash and refuse was noticed on the site. A stockpile of concrete rubble was <br />observed onsite as well as refuse/furniture. <br />CORRECTIVE ACTIONS: The operator shall submit a written notice to the Division with photo documentation, that the trash has <br />been removed from the site by the corrective action date. The operator shall also submit notification as to the purpose of the concrete <br />rubble per Rule 3.1.5(9). <br />CORRECTIVE ACTION DUE DATE: 11/12/10 <br />INSPECTION TOPIC: Hydrologic Balance <br />PROBLEM/POSSIBLE VIOLATION: Problem: The site is improperly impounding surface drainage/runoff. <br />CORRECTIVE ACTIONS: Operator should remove impediments to proper surface drainage by regrading to provide positive <br />drainage. <br />CORRECTIVE ACTION DUE DATE: 11/12/10 <br />INSPECTION TOPIC: Gen. Compliance With Mine Plan <br />PROBLEM/POSSIBLE VIOLATION: Problem: The current mine plan needs to be updated and clarified pursuant to C.R.S. 34-32.5- <br />112 (1)(c)(VI). The operator must provide sufficient information to describe or identify how the operator intends to conduct the <br />operation with respect to the active area being mined and reclamation timing. <br />CORRECTIVE ACTIONS: The operator should include revisions in the mining and reclamation plans in the pending permit <br />amendment to update and clarify the current approved mine plan to reflect existing and proposed activities. <br />CORRECTIVE ACTION DUE DATE: 12/12/10 <br />All of these issues may also be resolved by making the appropriate modifications to the permit amendment mining plan <br />submitted in response to the adequacy review process under a separate cover letter. <br />Page 1 of 4 <br />
