to indicate a permit area of 13 8.3 acres or revise the first page of the application to <br />indicate a 13 8.1 acre permit area and submit a new copy of this page of the application. <br />6.4.3 EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />2.) Please depict the "silt pit" on this map. <br />3.) For the Topographic Map submitted for this section. This map must be signed by the <br />registered land surveyor, professional engineer or the qualified person who prepared it. <br />Please indicate the scale the map was drawn at. This map is required to be drawn at a <br />scale no larger than Iinch = 50 feet nor smaller than 1 inch = 660 feet. Please include a <br />north arrow. <br />4.) The "Stockpile Map" and the "White Sand and Gravel Pit #1 Phase Map" must be <br />signed by the registered land surveyor, professional engineer or the qualified person who <br />prepared it. <br />6.4.4 EXHIBIT D - Mining Plan <br />5.) The mining plan indicates a second haul road will be used at the southeast end of the <br />permit area. One of the Exhibit C maps (Page 23) indicates this alternative road leaves <br />the permit area and travels south and then east to the state line. The plan indicates this <br />road is already in existence. Once the mining begins, will this alternative road be <br />improved for the mining operation? If so, this road must be included in the permit area <br />and the Applicant will need to amend the permit application to increase the permit area to <br />account for the road. <br />6.) The mining plan states, "Topsoil and overburden will occur with loaders, scrapers and <br />bulldozers or any other suitable equipment will be used". The Division assumes the <br />applicant is indicating topsoil and overburden will be stripped using the equipment <br />described? Please Confirm. <br />7.) Please indicate a depth the topsoil will be stripped? <br />8.) Please indicate a depth the overburden will be stripped? <br />9.) The mining plan indicates the applicant will mine below the groundwater table thus <br />exposing groundwater. If this will occur, the Applicant must obtain a temporary <br />substitute water supply plan and/or a well permit from the Office of the State Engineer. <br />Also, by exposing ground water the Division will need to hold a financial warranty <br />adequate to protect injuries to water rights should the permit be revoked and the financial <br />warranty forfeited. This will be discussed in further detail in Exhibit L. Please commit <br />to not exposing ground water until you have received approval from the Office of the <br />State Engineer and have submitted evidence to the Division of Reclamation, Mining and <br />Safety indicating such. <br />2
