This statement <br />provides for waiving any objection to issuance of a well permit by the State. By signing the waiver you <br />do not waive your right to protection of your well from injury due to mining. Upon our receipt of the <br />signed statement from you, we will forward it to the State Engineer's Office for processing with our well <br />permit application. <br />Aggregate Industries is committed to mitigating for impacts that may occur as a result of its operations <br />and respectfully requests your consideration in signing the within 600-Foot Well Spacing Statement. <br />Again, we would emphasize that signing of the form does not waive your right to protection of your well <br />from injury due to mining.
