The following records were not on file or need to be updated: <br />- ATFE Explosives Permit expired 8/1/2010 and is in the process of renewal <br />See attached DRMS Availability of Records <br />BACKFILL AND GRADING - RULE 4.14 <br />Backfilling and grading operations have been conducted at the 18RT Borehole site with all backfilling and grading <br />operations completed. <br />Backfilling and grading operations were being conducted at the bleeder shaft site at the time of the inspection. No <br />problems were noted during the operation. <br />HYDROLOGIC BALANCE - RULE 4.05 <br />Ponds B, C, D, E, F, G, 6NM, 18LT, and the Fish Creek Tipple Pond were observed during the inspection. The <br />6NM pond was dry at the time of the inspection with no pumping being conducted. <br />The Fish Creek Tipple Pond was discharging at the time of the inspection. A new CMP riser and CMP horizontal <br />outlet pipe has been installed at the pond with the V-notch weir moved -100 ft closer to the outlet pipe. The pond <br />embankment was stable, with no problems noted with the exception of the following: <br />- The embankment of the pond will require vegetation in the area of disturbance. <br />Ditches and culverts throughout the main facilities area and all outlying areas were inspected for compliance. <br />Adequate maintenance has been conducted on the following sites with no problems noted: <br />- The NWM Vent Fan culvert <br />- The E-1(b) culvert east of the shop <br />- The culvert to Pond B sump <br />- Culvert C-3 east of the batch weigh station has been addressed with the approved sediment control measures in <br />Number of Partial Inspection this Fiscal Year: 2 <br />Number of Complete Inspections this Fiscal Year: 2 <br />Page 2 of 6
