Colorado Stone Quarries <br />As Built Site Report <br />September 2010 <br />1. Introduction <br />Colorado Stone Quarries has a Minerals permit from the DRMS for the Yule Quarry under permit <br />number M-1999-058. Greg Lewicki and Associates was retained by Colorado Stone to make an <br />Amendment to the permit to add acreage and design a sediment control system for the quarry site. This <br />Amendment, AM-O1, was approved in June of 2010 and work commenced to complete all required <br />items by August 30th. During June, negotiations with contractors started and bids were received. The <br />work was started in July of 2010 and continued until the end of August. I, Greg Lewicki, P.E., was <br />integral in the design of the surface facilities and was involved in the proposals given to bidders. I also <br />actively worked with site personnel (of Colorado Stone) who worked on the site with the contractors <br />during the entire construction process. Greg Lewicki conducted inspections on May 21, July 29 and <br />September 16 of 2010. <br />2. Construction Items <br />The final inspection was conducted by me, Greg Lewicki, P.E. on September 16, 2010. Also present <br />were Cedric Beggar of Colorado Stone and Russ Means of the DRMS. <br />The following items were constructed as part of the Amendment work: <br />1) Installed erosion control fabric on 0.1 acres of steep slope below mine access road. Hydroseeded <br />and mulched slope, as required. The installation was performed carefully and was pinned down as well <br />as possible, considering the steepness of the slope and the presence of some rocks. <br />2) Installed 5 sediment sumps with emergency spillways and principal spillway pipes with valves. All <br />sumps were checked for volume requirements, adequacy of the emergency spillways, minor berm <br />construction, principal spillway pipes and valves. The changes in sump locations for Sumps 3 and 4 are <br />insignificant and do not require a redesign of the pond sizes. All ponds were found to be in compliance <br />except for the issues identified in the list shown in Section 4 of this report. <br />3) Installed collection ditches associated with the 5 sediment sumps. The collection ditches were <br />difficult to install in some places, since bedrock was close to the road surface. This was particularly <br />true of the collection ditch leading to Sump 5. All ditches were found to be in compliance except for <br />the issues identified in the list shown in Section 4 of this report. Some collection ditches needed to <br />cross the access road, and these crossings were accomplished with low water swales. <br />Yule Quarry As Built Report 2010
