Inspection Report <br />page 2 of 5 <br />the Division requested clarification from the applicant to ensure an understanding of site conditions prior to issuing <br />stormwater discharge permit certifications for the facilities. <br />Clarification took the form of a meeting with R2 Incorporated (consultant for Wildcat Mining Corporation) on May 27, <br />2010, and an inspection of the Idaho Mine facilities, the May Day Mine and Mill facilities, and all associated access roads <br />on June 30, 2010. <br />Observations and Findings associated with the June 30, 2010 inspections of the Idaho Mine facilities and the Mav Day <br />Mine and Mill facilities are addressed in this inspection report. Observations and Findings associated with the June 30, <br />2010 inspections of associated access roads (construction activities) are addressed in a separate inspection report, dated <br />August 19, 2010. <br />Note a: Division records indicate a stormwater discharge permit certification for the Idaho Mill was issued to Wildcat Mining <br />Corp on 10/17/2006 (permit certification COR04 0222). Industrial activities addressed in the permit application submitted for <br />this facility do not address any activities associated with the May Day Mine and Mill facilities. <br />Note b: Maps documenting information specific to Wildcat Mining Corporation operations were provided to the Division <br />inspector by R2 Incorporated during the inspection. These materials are referred to within this inspection report and attached. <br />The Division of Reclamation, Mining and Safety (DRMS) documented site conditions for portions of the industrial activities <br />described in this inspection report (inspection report dated June 3, 2010-May Day Mine). Relevant observations from these <br />inspections are referenced in this inspection report and attached. <br />Note c: The Colorado Water Quality Control Act requirements pertinent to this inspection report do not pre-empt, supersede, <br />or infringe on the lawful requirements and authority of federal agencies, state agencies, municipalities, counties, drainage <br />districts and local agencies. <br />Records Review <br />Note d: One location associated with the Idaho Mine application was inspected on June 30, 2010 (the Idaho Mine and Mill <br />Site). A map provided to the Division inspector by R2 Incorporated (see Attachment A) indicates the Idaho Mine and Mill Site <br />is comprised of the Payday Millsite and the Idaho Millsite. <br />Four locations associated with the MU Day Mine and Mill application were inspected on June 30, 2010 (see map provided to <br />the Division inspector by R2 Incorporated - Attachment B): <br />i. May Day No. 1 (Level 1); <br />ii. May Day No. 2 (Level 2); <br />iii. New portal at historic Chief portal location (not on map provided by R2 Incorporated); <br />iv. May Day No. 3 (Lamb). <br />Industrial activity conducted by Wildcat Mining Corporation was not observed at the Idaho Mine and Mill Site or at the May <br />Day No. 3 (Lamb). Therefore, findings contained in this inspection report are limited to observations made at May Day No. 1 <br />(Level 1); May Day No. 2 (Level 2); and the historic "Chief' portal. <br />1. Division records indicate one effective stormwater discharge permit certification (COR04 0222) and two pending <br />stormwater discharge permit applications (COR04 0252 and COR04 0253) for industrial activities conducted by Wildcat <br />Mining Corporation (see Background and Note a, above). <br />Because of potential overlap of coverage between COR04 0222 and COR04 0252 (pending), the Division expects <br />Wildcat Mining Corporation to review existing and requested stormwater discharge permit coverage, and provide the <br />Division with a determination of the permits it wishes to become/remain effective. If Wildcat Mining Corporation <br />decides to inactive an effective stormwater discharge permit or withdrawal an application, the Division expects that <br />Wildcat Mining Corporation will submit the appropriate paperwork to the Division.
