No.: COR04 0252 (pending) Date: 06/30/2010 <br /> COR04 0253 (pending) <br />Facility: Idaho Mine and May Day Mine and Mill Industrial Type: Industrial Receiving Water: Little Deadwood Gulch <br />site and La Plata River <br />Facility Address: North and north-west of unincorporated Mayday village, La Plata County, Colorado <br />Persons present: Mike Clements (Wildcat Mining Corporation - Owner); Ginny King and George Robinson (R Incorporated); Wally <br />Erickson and Steve Shuey (Division of Reclamation, Mining and Safety); Andrew Neuhart, Kathy Rosow and Erin Scott (CDPHE - <br />WQCD). <br />Facility Representative(s)/Title(s): Mike Clements (Owner) Inspector(s): Kathy Rosow CDPHE (WQCD) <br />Inspection Findings <br />The Water Quality Control Division (Division) inspector held a closing conference at the conclusion of the inspection. During <br />the closing conference, the inspector reviewed the inspection findings with the person present (Ms. Ginny King-R2 <br />Incorporated) and communicated the Division's expectation that all findings be corrected. <br />Background <br />Two applications for permit coverage under the CDPS General Permit for Stormwater Discharges Associated with Metal <br />Mining Operations and Mine-Waste Remediation (industrial stormwater discharge permit) were received from Wildcat <br />Mining Corporation on January 12, 2010. Although the latitude/longitude and site maps were different for the two <br />applications, the Facility Name on both applications was the May Day Mine and Mill. The Division requested clarification <br />from the applicant. <br />Two new applications and associated documentation were received from Wildcat Mining Corporation on April 15, 2010. <br />Facility names on the applications indicated two different facilities: the Idaho Mine and the May Day Mine and Mill. <br />Documentation received with the applications included: <br />• Stormwater Management Plan, Idaho-Matey; Sakura Engineering, Paul Martin P.E., December 17, 2008. <br />This document addressed the Idaho-Mayday Mining and Milling site and the Idaho-Mayday Access Road. The <br />SWMP discussed storage and processing of material from the Mayday Mine, and the Idaho-Mayday Access Road off <br />County Road 124. The location map included in the SWMP did not correlate with the site map provided in the <br />applications, appeared to indicate activities at both the Idaho Mine and May Day Mine and Mill locations, and <br />discussed an access road (Idaho-Mayday Access Road) not included in the description of industrial activities on either <br />application submitted to the Division. <br />• Drainage Report & Terrain Management Idaho-Mayday Access Road; Sakura Engineering <br />Paul Martin P <br />E <br />, <br />. <br />., <br />December 18, 2008. <br />This document addressed the Idaho-Mayday Access Road, and states the access road construction is less than I acre. <br />The access road discussed in the document was not included in the description of industrial activities on either <br />application submitted to the Division. <br />• Stormwater Management Plan, Idaho; unknown author, December 29, 2009. <br />This document addressed the Idaho Mining and Milling site and the Idaho Access Road.
