Stormwater Inspection Report
Permittee: Wildcat Mining Corporation Cert. No.: COR04 0252 (pending) Date: 06/30/2010
COR04 0253 (pending)
Facility: Idaho Mine and May Day Mine and Mill Industrial Type: Industrial Receiving Water: Little Deadwood Gulch
site and La Plata River
Facility Address: North and north-west of unincorporated Mayday village, La Plata County, Colorado
Persons present: Mike Clements (Wildcat Mining Corporation - Owner); Ginny King and George Robinson (R Incorporated); Wally
Erickson and Steve Shuey (Division of Reclamation, Mining and Safety); Andrew Neuhart, Kathy Rosow and Erin Scott (CDPHE -
WQCD).
Facility Representative(s)/Title(s): Mike Clements (Owner) Inspector(s): Kathy Rosow CDPHE (WQCD)
Inspection Findings
The Water Quality Control Division (Division) inspector held a closing conference at the conclusion of the inspection. During
the closing conference, the inspector reviewed the inspection findings with the person present (Ms. Ginny King-R2
Incorporated) and communicated the Division's expectation that all findings be corrected.
Background
Two applications for permit coverage under the CDPS General Permit for Stormwater Discharges Associated with Metal
Mining Operations and Mine-Waste Remediation (industrial stormwater discharge permit) were received from Wildcat
Mining Corporation on January 12, 2010. Although the latitude/longitude and site maps were different for the two
applications, the Facility Name on both applications was the May Day Mine and Mill. The Division requested clarification
from the applicant.
Two new applications and associated documentation were received from Wildcat Mining Corporation on April 15, 2010.
Facility names on the applications indicated two different facilities: the Idaho Mine and the May Day Mine and Mill.
Documentation received with the applications included:
• Stormwater Management Plan, Idaho-Matey; Sakura Engineering, Paul Martin P.E., December 17, 2008.
This document addressed the Idaho-Mayday Mining and Milling site and the Idaho-Mayday Access Road. The
SWMP discussed storage and processing of material from the Mayday Mine, and the Idaho-Mayday Access Road off
County Road 124. The location map included in the SWMP did not correlate with the site map provided in the
applications, appeared to indicate activities at both the Idaho Mine and May Day Mine and Mill locations, and
discussed an access road (Idaho-Mayday Access Road) not included in the description of industrial activities on either
application submitted to the Division.
• Drainage Report & Terrain Management Idaho-Mayday Access Road; Sakura Engineering
Paul Martin P
E
,
.
.,
December 18, 2008.
This document addressed the Idaho-Mayday Access Road, and states the access road construction is less than I acre.
The access road discussed in the document was not included in the description of industrial activities on either
application submitted to the Division.
• Stormwater Management Plan, Idaho; unknown author, December 29, 2009.
This document addressed the Idaho Mining and Milling site and the Idaho Access Road. The SWMP discussed
storage and processing of material from the Idaho Mine, and the Idaho Access Road off County Road 124. The
access road discussed in the SWMP was not included in the description of industrial activities on either application
submitted to the Division.
Because the information in the applications provided to the Division on April 15, 2010 (Idaho Mine and the May Day
Mine and Mill), did not correlate in content or nomenclature with the three documents submitted with the applications,
