Compliance With Mine Plan NA - Support Facilities Not On-site <br />NA - Other NA - Special Categories Of Mining <br />NA - Processing Waste R -Topsoil <br />COMMENTS <br />AIR RESOURCE PROTECTION - Rule 4.17: <br />Roads were a source of significant dust on site during the inspection due to the recent lack of precipitation <br />and windy conditions. This was most notable in the Oak Gulch drainage where soil has recently been disturbed by <br />gully-repair activities. It did not appear that a significant amount of dust was leaving the site. <br />EXCESS SPOIL and DEVELOPMENT WASTE - Rule 4.09 F1 ?Placement; Drainage Control; Surface <br />Stabilization: <br />Construction of Horse Gulch Fill was observed. Topsoil has been placed on the constructed slopes <br />between the toe of the slope and the first bench and between the first and second benches. TMI staff indicated that <br />seeding of the area will be performed in the next week. No rip rap has been placed in the excavated side drains. <br />HYDROLOGIC BALANCE - Rule 4.05 El ?Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage - Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />A track hoe was cleaning a dozer basin in the East Pyeatt drainage at the time of the inspection. <br />Several sediment ponds on the site were inspected, and the following maintenance issues were observed: <br />• Sediment level markers appear to be missing or under water for West Pyeatt #1, Middle Flume #3, and No <br />Name #2. <br />Sediment removal is needed from Oak #1. <br />Animal burrows continue to be a nuisance (but not yet a significant problem) at East Flume #1. <br />Also on the East Flume #1 dam, a tamarisk plant is growing. <br />The inspection indicated no water quality problems, such as oil and grease or turbid water, at any of the pond <br />discharge points. <br />Number of Partial Inspection this Fiscal Year: 2 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 2 of 8
