<br />6 <br />approved by the White River Field Office Manager. Item A illustrates the minimum requirements for plugging drill <br />holes. <br />One copy of the drill-hole data and related reports will be submitted to the White River Field Office Manager within <br />30 days after each calendar year. The requirements for submission of drilling and abandonment information are given to <br />the licensee herewith as Item B. The licensee's location map must show access trail locations and exact hole locations <br />appropriately identified by number or letter. Submitted data must be tied to the hole number or letter designations. All <br />holes will be geophysically logged with natural gamma, density, caliper, or resistivity logs. Geophysical logs must run <br />on each hole; unless the hole conditions jeopardize the safety of the geophysical tools. <br />One copy of all drill hole information, including logs, coal analyses, desorption tests, etc., and location map of <br />surveyed locations, must be submitted to BLM no later than within 30 days after the calendar year. In addition, submit a <br />description or illustration of the method used to plug each hole or completion as monitoring well. <br />This license consists of the terms and conditions in Sec. 1 through 14 on six pages, of which this is the sixth page and <br />the attached FONSI, Item A which illustrates minimum requirements for plugging drill holes, and Item B which outlines <br />the requirements for submission of drilling and abandonment information. <br />BLUE MOUNTAIN ENERGY, INC.
