Compliance With Mine Plan R - Support Facilities Not On-site <br />NA - Other NA - Special Categories Of Mining <br />R - Processing Waste Y -Topsoil <br />COMMENTS <br />This was a complete inspection conducted by Janet Binns of CDRMS. Mr. George Patterson of EFCI provided <br />access to the mine records at the EFCI office, but did not accompany the inspector to the mine site. The gate to <br />the mine site remains appropriately locked. The state inspector has keys to the lock, provided by EFCI. The mine <br />site was dry, though Mr. Patterson informed the inspector that the region had received periodic rainstorms <br />throughout the preceding week. No meteorological records were available at the mine site. <br />AVAILABILITY OF RECORDS - Rule 5.02.4(1): <br />Records were checked at the Energy Fuels office, located at 115 E Main street in Florence, CO. Mr. <br />George Patterson met the inspector at the office and provided access to the records. Mr. Patterson did not <br />accompany the inspector to the mine site. <br />Records were complete and up to date With the Exception of 1 st and 2nd quarter sediment pond inspections and <br />waste pile reports. Energy Fuels needs to ensure that records are maintained and are up to date. Mr. Patterson <br />stated that a separate contractor completes the sediment pond and waste pile inspections and that EF has not <br />received the reports. <br />HYDROLOGIC BALANCE - Rule 4.05 ? ? Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage - Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The mine site was dry during the inspection. Conversation with Mr. Patterson indicated that the area had <br />been receiving monsoon moisture over the past several weeks. Energy Fuels no longer maintains a rain gauge, on <br />site, so there is no accurate measurement of recent precipitation events at the mine site. Both ponds 4 & 5 were <br />dry and well vegetated. <br />Ditches were functional. No problems were encountered on the mine site.
