Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Phase 4B/C Triangle Liner Certification <br />August 10, 2010 <br />AMEC provided CQA laboratory and field testing, and monitored earthworks construction <br />activities as follows: <br />¦ Site grading, including slope re-contouring and fill placement, lift thickness, moisture <br />conditioning, temperatures, and compactive effort. No Structural Fill (SF) was required <br />since the earthworks consisted primarily of minor re-sloping; <br />¦ Subgrade preparation to confirm that the subgrade surface was unyielding, free of <br />organic matter and other unsuitable materials, and that the subgrade surface was <br />compacted prior to Soil Liner Fill (SLF) placement. <br />¦ AMEC visually monitored the placement and compaction of SLF including observation of <br />depth verification, particle size, moisture conditioning, and compaction of the fill. AMEC <br />performed in-place density and moisture content testing, and performed CQA laboratory <br />testing of grain size, moisture content, Atterberg limits, compaction, and permeability. <br />AMEC also provided visual monitoring of the SLF surface preparation and moisture <br />conditioning for geomembrane deployment, including verification that the surface was <br />proof-rolled, compacted, and hand-worked so as to be free of desiccation cracks, abrupt <br />grade changes, and sharp stones or objects. AMEC also observed the backfill and <br />compaction of the anchor trenches. <br />¦ AMEC visually monitored the placement of Drain Cover Fill (DCF) within certification <br />area, including verification of lift thickness and grade control, identification and removal <br />of deleterious materials, and verification that care was exercised in fill placement to <br />avoid geomembrane damage. AMEC performed CQA laboratory testing of grain size <br />and Atterberg limits. <br />¦ AMEC monitored the ambient air temperatures and the temperatures of all fills in <br />accordance with project specifications. <br />1.2.2 CQA/CQC Monitoring of Geosynthetics Installation <br />AMEC provided CQA monitoring of geosynthetics used for certification of the Phase 4B/4C <br />Triangle. CQC services provided by AMEC consisted of performing geosynthetics inventory and <br />checking quality control certifications prior to geosynthetics deployment. AMEC performed <br />geosynthetics CQA activities in accordance with the CQA Plan.
