Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Phase 4B/C Triangle Liner Certification <br />August 10, 2010 <br />¦ Performing conformance testing of geomembrane; <br />¦ Measuring geomembrane panel thickness and confirming adequate panel overlap; <br />¦ Visually inspecting geomembrane sheet quality, sheet grinding, seam alignment, and <br />continuity; and <br />¦ Retaining archive samples of geomembrane destructive tests and trial seams. <br />1.3 Project Coordination <br />AMEC CQC and CQA activities were directed by the AMEC Resident Engineer. The AMEC <br />Resident Engineer for the period was Mr. Joseph D. Hickey. The Certifying Engineer, <br />Mr. John F. Lupo, Ph.D., P.E. or his representatives (Mr. Derek Wittwer, P.E., Project Manager) <br />made periodic site visits to resolve technical issues and to review AMEC activities. <br />Representatives of CC&V, Ames, Foresight West (QA Surveyor), and AMEC conducted daily, <br />weekly, and preconstruction coordination meetings throughout the construction period. <br />1.3.1 Daily Construction Summary Reports <br />AMEC prepared daily construction summary reports as well as photographic documentation <br />(Appendix A) throughout the project to document the key elements of construction and the CQA <br />activities that occurred. On-site AMEC CQA personnel, the Certifying Engineer and the Project <br />Engineers discussed construction activities on a regular basis. In addition, field personnel <br />prepared daily observation reports. AMEC submitted these daily reports and a daily summary <br />report to CC&V. Daily reports are not included in this report but were available for Colorado <br />Division of Reclamation, Mining and Safety (DRMS) review during the course of the project. <br />The AMEC staff schedule for the duration of the project can be found in Appendix B. <br />1.3.2 Weekly Construction Summary Reports <br />AMEC prepared weekly construction summary reports which were submitted to CC&V and to <br />the DRMS. Weekly Reports from the period of May 6 through May 15, 2010 are presented in <br />Appendix C. <br />1.3.3 Project Specifications <br />All work was performed in accordance with the design plans and specifications in Amendment <br />Nos. 8 and 9 to Mining Permit M-1980-244.
