Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Phase 4B/C Triangle Liner Certification <br />August 10, 2010 <br />6.1.1 Conformance Testing <br />Geomembrane manufactured by Poly-Flex. An AMEC representative performed geomembrane <br />conformance sampling at the Poly-Flex plant in Grand Prairie, Texas. Samples were sent to <br />TRI/Environmental, Inc. geosynthetic laboratory in Austin, Texas, for conformance testing. <br />Geomembrane rolls were not used on the project unless approved by AMEC based on <br />conformance test results. <br />Prior to the geomembrane arrival to site, TRI performed one (1) conformance tests at a <br />frequency of approximately one test per 10,000 square feet, exceeding the minimum specified <br />testing frequency of one test per 150,000 square feet. Conformance 100-mil single sided <br />texture LLDPE geomembrane used to construct the Phase 413/4C Triangle liner system met <br />specifications. Conformance test results are presented in Appendix I. <br />6.1.2 Deployment Monitoring <br />Geomembrane panel deployment monitoring performed by AMEC included: <br />¦ Observation of panel surface condition and panel overlap; <br />¦ Assignment of panel numbers; <br />¦ Verification of factory roll number and receipt of complete certification prior to allowing <br />deployment; <br />¦ Measurement of sheet thickness; <br />¦ Documentation of deployed panel lengths; <br />¦ Development of panel layout drawing; and <br />¦ Observation of VLF tie-in. <br />Prior to the deployment of geomembrane, AMEC and Lonestar verified that the subgrade was <br />free of soft spots, protrusions, angular particles, abrupt grade changes, and loose soil. AMEC <br />and Lonestar verified that the SLF surface had been proof-rolled, and that the SLF surface was <br />free of desiccation cracks. Where the surface of the SLF did not meet specifications, Ames <br />repaired the area and re-compacted the surface. Lonestar provided written certification of <br />geomembrane subgrade acceptance. The subgrade acceptance certificates are presented in <br />Appendix E. <br />AMEC marked the panel designation and roll number directly on each panel after it was <br />deployed.
