Field personnel collecting the samples will be responsible for custody until the samples <br />are delivered to the laboratory or relinquished to a commercial shipping company. <br />Sample transfer requires the individuals relinquishing and receiving the samples to sign, <br />date, and note the time of transfer on the chain-of-custody forms. Common carriers (e.g., <br />Federal Express) are not expected to sign the chain-of-custody forms. However, the bill <br />of lading or air bill becomes part of the chain-of-custody record when a common carrier <br />is used to transport the samples. The chain-of-custody is considered complete after the <br />analytical laboratory accepts custody of the samples (acceptance of custody is indicated <br />by signature on the chain-of-custody form). A copy of the chain-of-custody record will <br />be maintained along with other field records. <br />• <br />6.2.2 Quality Assurance/Quality Control <br />The following quality assurance and quality control (QA/QC) actions will be <br />implemented to minimize the potential for biasing the analytical results by laboratory <br />preparation, sampling, and transport activities. <br />• Fieldwork will be performed by qualified and trained personnel including the <br />site owner or company personnel. Persons performing the sampling will be <br />trained by a professional qualified in groundwater sampling. <br />• Samples will be analyzed by a qualified laboratory. The laboratory will use <br />appropriate chain of custody, analytical, and QA/QC procedures. <br />• A trip blank for VOC analysis may be included in sampling events at a <br />frequency of one trip blank per year depending on whether questions arise <br />relative to the quality of the analytical data. <br />• Equipment blanks and field duplicates will not be collected on a routine basis <br />because disposable or dedicated sampling equipment will be used; however, <br />they may be prepared and analyzed if questions arise relative to the quality of <br />the analytical data. <br />• The full laboratory report, including laboratory QC data, will be attached to the <br />monitoring reports submitted by the owner. <br />• The laboratory results will be validated using standard methods. <br />6.2.3 Groundwater Well Maintenance <br />The condition of the groundwater monitoring system will be inspected during each <br />monitoring event. The results of the inspection will be documented and any deficiencies <br />will be remedied within 60 days of the inspection or at a later date as approved by <br />CDPHE. If deficiencies, malfunctions or deteriorations are observed at other times, such <br />deficiencies will also be documented and remedied within 60 days of discovery or at a <br />later date as approved by CDPHE. <br />AFS Frei-Henderson Inert Fill <br />MOLEN & ASSOCIATES, LLC <br />Page 19 of 23
