a) The Operator has submitted Information Sheets on two different water control <br />structures: Inlet Water Level Control Structure and Inline Water Level Control <br />Structure. Please specify which structure will be installed. <br />b) Please specify the dimension of the earthen dam (length, width, height and total <br />volume). This information is required to calculate the cost to remove this structure <br />upon the completion of mining and stabilization of the pit. <br />c) Provide a maintenance plan describing how the erosion control structure will be <br />inspected, maintained and repaired if necessary. <br />d) Provide a revised Mining Plan Map depicting the exact location of the dam. <br />e) The Operator stated vegetation will be used to help prevent wind and water erosion. <br />Will the earthen dam be seeded? If so, please specify the type of seed to be used and <br />the application rate (PLS/acre). <br />f) The Operator stated riprap aprons will be placed on both sides of the dam. Please <br />specify the dimension of each of the riprap aprons. in addition, specify the type of <br />riprap to be used and the source. <br />g) Please specify the source of the material to be used in constructing the earthen dam. <br />In addition, describe how the dam will be constructed. <br />3. The Operator stated the second discharge point at the top of the main drainage channel was <br />closed. Please describe how this opening was closed. <br />4. If the Operator constructs a dam, will the straw bales remain in the main drainage channel? <br />5. The Operator has not addressed how water will be routed into the drainage channels. There <br />are currently two discharge points from the site; the main drainage channel which flows to <br />the south and a secondary drainage channel which flows from the northeast corner to the <br />southeast. Will water be routed to a single drainage point or will it be allowed to discharge <br />from both points? Please describe how stormwater will be controlled and directed offsite. <br />This concludes the Division's adequacy review of this application. Please remember that the <br />decision date for this application is August 19, 2010. As previously mentioned if you are unable <br />to provide satisfactory responses to any inadequacies prior to this date, it will be your <br />responsibility to request an extension of time to allow for continued review of this <br />application.
