PERMIT #: M-2007-031
INSPECTOR'S INITIALS: BMK
INSPECTION DATE: June 15, 2010
OBSERVATIONS
The Division conducted a joint moitoring inspection of the site with Stephanie Carter of BLM.The site was not active at
the time of the inspection. Opeartor was notified but unable to attend. The entire permit acre of 1.7 acres was walked and
inspected.Permit sign was pterly posted at the access to the property.
A couple of ront end loaders were parked near the main pit area. Both the two western pits were holding a small amount
of water from the recent pricipitation the area received. The upper pit was receving some surface water seepage at the time
of the inspection. Since the site is with in the channel of the gulch, even with the diversion, one expects water to seep in
from the upper water shed for the operation.
Afew 5 gallon buckets, some empty some still holding what appeared to be spent solvent and oil were noted. Opertor must
collect all these five gallon buckets and place them in a protected area.
The road high wall that is utilized to acess the pit apperda a bit steepr than what is allowed and recommended in the
permit. Operator must lower the hih wall slopes to at least 2:1 .
The current warrnty of $6,352.00 is more than adeqaute to cover the cost of recalmation. With the exception of the above
minor problem, the operation is in compliance with the approved reclamation plan
GENERAL INSPECTION TOPICS
The following list identifies the environmental and permit parameters inspected and gives a categorical evaluation of each
RECORDS----------------------------------- Y (FN) FINANCIAL WARRANTY-------- Y (RD) ROADS------------------ Y
HYDROLOGIC BALANCE------------- PB (BG) BACKFILL & GRADING---------- PB (EX) EXPLOSIVES--------- NA
PROCESSING WASTE/TAILING---- NA (SF) PROCESSING FACILITIES------- NA (TS) TOPSOIL---------------- Y
GENL MINE PLAN COMPLIANCE- Y (FW) FISH & WILDLIFE----------------- Y (RV) REVEGETATION---- NA
SIGNS AND MARKERS----------------- Y (SP) STORM WATER MGT PLAN---- NA (SB) COMPLETE INSP---- NA
OVERBURDEN/DEV.
