OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Backfilling & Grading <br />PROBLEM/POSSIBLE VIOLATION: Problem: A small amount of 5 gallon containers were improperly stored on site. ? ? The access <br />road high wall to the active pit area appears to be slightly steeper than what is recommended? ? ? ? <br />CORRECTIVE ACTIONS: The operator shall immediately store all 5 gallon fuel containers and solvent containers and store them <br />properly, by placing them in a protected area. ? <br />The opertaor must cut tthe high wall to at least 2:1 <br />CORRECTIVE ACTION DUE DATE: 7/30/10 <br />Page 1 of 2
