Pinecla <br />Director <br />Re: Vick Borrow Pit, File No. M-2010-048, Receipt of Special 111 Construction Materials Reclamation <br />Permit Application Package <br />Dear Mr. Bell: <br />On June 25, 2010, the Division of Reclamation, Mining and Safety (Division) received your Special 111 <br />Construction Materials Reclamation Permit Application package for the Vick Borrow Pit, File No. M-2010-048. <br />9-K-ifliminary review of the information received determined the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />Please provide completed copies, signed and dated, of Addendum 1- Notice as required by Rule 1.6.2(1) <br />(b). <br />Application Form <br />STATE OF COLORADO <br />2. Section 34-32.5-111(2)(a) of the Colorado Land Reclamation Act for the Extraction of Construction <br />Materials states an operator shall apply for a special permit by filing a written application with the board <br />on forms provided by the board for such purpose. The Division does not consider the application form <br />submitted by the applicant as a form provided by the board. Please resubmit the Construction Materials <br />Special (111) Operation Reclamation Permit Application on an approved form. Approved forms are <br />available at www.mining.state.co.us. <br />Rule 6.2.1(2) requires maps must be prepared and signed by a registered land surveyor, professional <br />engineer, or other qualified person. Please resubmit signed copies of the maps to the Division. <br />6.3.1 Exhibit A - Legal Description and Location Map <br />4. Rule 6.3.1(1) (b) requires the location of the main entrance to the mine site be reported as latitude and <br />longitude, or the Universal Transverse Mercator (UTM) Grid as determined from a USGS topographic <br />map. Please provide the location of the main entrance to the mine site on Exhibit A. <br />5. Rule 6.3.1(2) requires the main entrance to the mine site shall be located based on a USGS Topographic <br />map showing latitude and longitude or Universal Transverse Mercator (UTM). The operator will need <br />to specify coordinates of latitude and longitude in degrees, minutes and seconds or in decimal degrees <br />to an accuracy of at least five (5) decimal places (e.g., latitude 37.12345 N, longitude 104.45678 W).
