amec* <br />Earth & Environmental <br />C) Geomembrane Delivery <br />1) VLF <br />Three loads (12 rolls each) of 100mil smooth geomembrane and one load (11 rolls) of 80mil <br />LLDPE single sided textured geomembrane were delivered to site during the week for a total <br />of 47 rolls. All loads were delivered on flat bed trucks in good condition. The rolls were <br />unloaded using a CAT 966 front-end loader with forks attached. The geomembrane was placed <br />on three pieces of 4" HDPE pipe, staked two rolls high. An AMEC field representative was <br />present: at all times during unloading of the geomembrane. The AMEC representative also <br />inventoried the geomembrane, as it was unloaded. <br />D) Anchor Trench <br />1) VLF <br />Excavation of the anchor trench between stations B7+50 (+/-) and B10+00 (+/-) continued <br />using a John Deere 35D excavator with assistance from laborers. The anchor trench was <br />inspected by an AMEC filed representative and was found to be constructed according to <br />design drawings and project specifications. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed structural fill placement within the Phase 5 VLF fill area. Ambient temperatures were <br />monitored during fill placement. <br />AMEC observed, performed necessary testing, obtained moisture samples, and assisted in the placement <br />and moisture conditioning of SLF material within the Phase 5 VLF. <br />AMEC inspected and approved subgrade for geomembrane deployment within the Phase 5 VLF. <br />AMEC observed, logged data, and assisted in the installation of geomembrane within the Phase 5 VLF. <br />AMEC inspected and accepted secondary geomembrane within the Phase 5 VLF. <br />AMEC observed construction of the PSSA ore slope. <br />AMEC observed LVSCF placement within the Phase 5 VLF constantly. <br />AMEC observed the placement of topsoil at the downstream slope of the South Toe Berm. <br />AMEC observed stripping of topsoil at the East Cresson Clay Borrow. <br />AMEC observed, obtained samples, and assisted in the process of SLF material at the East Cresson Clay <br />Borrow. <br />VLF Weekly Report WE 06.12.10.1)
