19. Section 6.3.3(1)(0) of AM-01 indicates that explosives will be utilized underground, provides a <br />summary of information required for blasting reports, and commits to conducting pre-blast <br />surveys for all structures located within 0.5 mile of the proposed shot. Attachment C-7 contains a <br />letter from Franklin Drilling and Blasting, Inc., dated February 24, 2010, and a statement from <br />Terry Morris Engineering, which is not signed or dated. Please note that.engineering reports must <br />be signed and dated by the qualified person who prepared the report. if the qualified person who <br />prepared the report is a professional engineer, such report should bear the certification statement <br />and seal from that licensed individual. Additionally, the statements from Franklin Drilling and <br />Blasting and Terry Morris Engineering discuss the effects of underground blasting at depths of <br />1,000 feet or greater, but do not address the potential effects of blasting at or nearby the surface. <br />Near surface blasting has occurred at the site as recently as September 3, 2009, and the <br />application must address such activity. Please provide the information required by Rules <br />6.3.3(1)(0) and 6.5(4). <br />20. Pursuant to Rule 6.3.3(2)(a), please provide operation details for the milling plant including tank <br />capacities and operating solution volumes. <br />21. Pursuant to Rule 6.3.3(2)(d), please provide engineering designs and drawings for the steel set <br />portal entry at Level 1, as necessary for the Office to review and approve the structure. <br />22. Pursuant to Rule 6.3.3(2)(d), please provide specific design details for the tail transport systems <br />and specific disposal location for the tailings. <br />Exhibit D, Reclamation Plan, Rule 6.3.4 <br />23. Pursuant to Rule 6.3.4(1)(b), please provide a justification that supports final slopes being steeper <br />than 3H:1V and which demonstrates compliance with the performance standards of Rule 3.1. <br />24. Please describe the specific steps in the procedure for dismantling the mill facility and steel set <br />entry at the Level 1 area. <br />25. The proposed reclamation plans indicates that some of the milling structures will be removed and <br />some will be buried at the portal. Please specify which structures will be removed from site and <br />which structures will be buried on-site. For the itemized list of structures to be buried on site, <br />please describe the measures taken to minimize disturbances to the hydrologic balance, prevent <br />off-site damange, and provide for a stable configuration consistent with the proposed future land <br />use, as required under Rule 6.3.4(1)(e). <br />Exhibit E, Map, Rule 6.3.5 <br />26. Although required by Rule 6.3.5(1), none of the maps submitted with AM-01 show any section <br />corners adjacent to the proposed operation. Please submit new maps which satisfy all map <br />requirements of.Rules 6.2.1(2) and 6.3.5 <br />5
