• Outline and label all major surface features to be used in connection with the proposed <br />operation such as existing and proposed roads, pit boundary, topsoil stockpiles, <br />overburden stockpiles, buildings, ponds, processing plants, etc. <br />• Indicate the direction that construction material extraction will proceed. <br />• Note the location of any significant, valuable, and permanent man-made structures <br />within 200 feet of the affected area. The gas line and well and overhead power line must <br />be portrayed on the Mining Plan Map. <br />• Outline any existing disturbance within the permit boundary. <br />At a minimum the Reclamation Plan Map shall contain the following information: <br />• Show the proposed topography of the area with contour lines of sufficient detail to _l <br />portray the direction and rate of slope of all reclaimed lands. -? <br />• Outline the final configuration of the ponds. <br />• State the average thickness of replaced overburden by reclamation area or phase <br />• State the average thickness of replaced topsoil by reclamation area or phase. <br />Mining Plan <br />During the meeting on June 7, 2010, the Operator informed the Division of plans to expand the <br />southwest pond in an easterly direction. The Mining Plan must be revised to reflect this change. <br />The Mining Plan shall supply the following information: <br />• Describe the size of areas to be worked at any one time. <br />• Provide an estimate of the periods of time which will be required for the various stages or <br />phases of the operation. <br />• Provide a description of the size and location of each area to be worked during each <br />phase. <br />• Outline the sequence in which each stage or phase of the operation will be carried out. <br />Permanent Man-made Structures <br />During the inspection conducted on March 16, 2010, the Division identified several permanent <br />man-made structures within 200 feet of the affected area. The structures include a gas well, gas <br />line, and an overhead power line. The Division has determined the Permittee does not have a <br />structure agreement with the utilities. The Permittee shall supply a notarized letter, on utility <br />letterhead, from the owner(s) of the utility that the mining and reclamation activities shall have <br />"no negative impact" on their utility. If the Permittee cannot obtain a letter from the utility, then <br />the Permittee shall provide an adequate engineering evaluation that demonstrates that such <br />structure shall not be damaged by activities occurring at the mining operation. In addition, the <br />permanent man-made structures were not portrayed on the Mining Plan Map. The revised <br />Mining Plan Map shall portray the structures and identify the owners of the structures.
