5
Exhibit S Permanent Man Made Structures
The damage agreements were signed by Four Corners and the owner of the facilities did not
sign and return the agreement and therefore the engineering calculation of slope stability was
provided. The half completed damage agreements were included in the application to show
that the first part of rule 6.4.19(b) was followed. Since the damage agreements were not
returned we proceeded with the second part of rule 6.4.19(6) and provided an engineering
slope stability study. It is believed that no further action is required for man made structures
within 200 feet of the permit boundary.
Please call Matt Carnahan with Four Corners Materials if there are any questions or
additional information needed.
Sincere
Lly
tt C Four Corners Materials & Ryan Ellis, Greg Lewicki and Associates
Ma
encl.
ec, w/out encl.
