(Page 3) <br />MINE ID # OR PROSPECTING ID #: mod-200.5-039 _ <br />INSPECTION DATE: 4/10/10 INSPECTOR=S INITIALS: MAC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The topsoil stockpiles do not have. established vegetation on them and are susceptible to 5 /28 /10 <br />erosion. Rule 3.1.9(1) states that if topsoil is not replaced into the backfill area within a time <br />short enough to avoid deterioration of the topsoil, vegetative cover or other means shall <br />be employed so that the topsoil is protected from erosion. <br />PB or PV: PB_ <br />TOPIC(S): IS <br />Corrective actions are to: <br />The Operator shall submit a Technical Revision to the permit along with the appropriate <br />revision fee of $216.00 to the Division on or before May 28, 2010. The Operator shall <br />demonstrate compliance by submitting seed tags, a bill of sale or photographs of seeding <br />activities.
