iv. Specify the application method for grass seeding. If the seed is to be broadcast, the <br />application rate shall be twice the rate required for seed drilling. <br />V. If mulch is applied, specify the kind to be used, the crimping method, and rate of <br />application. <br />3. The Applicant has stated reclamation will be concurrent with mining. Please specify the <br />maximum amount of disturbance that will occur on the site at any given time. How much <br />overburden (cubic yards) will be backfilled into the pit to create 3H:I V slopes? How much <br />topsoil (cubic yards) will be replaced on top of the overburden? <br />6.3.5 Exhibit E - Map <br />4. In accordance with Rule 6.3.5(2)(a), label the permit boundary on the Mining Plan Map as it <br />is deser1UCU in ExI1101L h - 1 gUl 1JGSQ11jJL1011. <br />5. In accordance with Rule 6.3.5(2)(b), the Mining Plan Map shall contain the names of <br />owner(s) of record of the surface of the affected area and of the land within 200 feet of the <br />affected area. In addition, identify the owner of the substance to be mined and the type of <br />structure and owners of record of any permanent man-made structures within 200 feet of the <br />affected area. <br />6. In accordance with Rule 6.3.5(2)(c), the Mining Plan Map shall contain labels of any topsoil <br />stockpiles, overburden stockpiles and product stockpiles. <br />6.3.6 Exhibit F - List of Other Permits and Licenses Required <br />7. The Applicant has stated no other permits will be sought for this operation. Please indicate if <br />the Applicant has contacted the Colorado Department of Public Health and Environment in <br />regard to the need for a storm water discharge permit. <br />7.3.12 Exhibit L - Permanent Man-Made Structures <br />8. The Applicant has stated there are no man-made structures within 200 feet of the affected <br />area. The Division considers the road, which bisects the permit boundary, to be a permanent <br />man-made structure. The Applicant must provide a notarized agreement between the <br />Applicant and the Person(s) having an interest in the structure, that the Applicant is to <br />provide compensation for any damage to the structure. If such an agreement cannot be <br />reached, the Applicant shall provide an engineering evaluation that demonstrates that such <br />structure shall not be damaged by mining activities occurring at the mining operation. In <br />addition to the above listed options, the Applicant may also choose to establish a mining <br />setback distance of greater than 200 feet from the road. If the Applicant chooses to establish <br />a setback distance, the setback will need to be depicted on the Mining Plan Map.
