Enclosed is a brief description and Purchase Order Quotation Form for the Last Call at
Fruita Mine Project (Permit No. C- 1981 -015).
Site Showing
A site showing will be held on September 1, 2009 at 8:00 a.m at the City Market in
Fruita, Colorado.
Quote Submission and Evaluations 0
The price quotation shall be submitted by 3:30 p.m. on September y0; 2009 by fax, mail,
or in person to:

Telephoned bids will not be accepted.
The attached quotation form must be submitted. Award will be made if the amount bid is
within the Project budget estimate, and if the equipment and personnel offered are
deemed sufficient by the Division to accomplish the Project within the specified time of
completion.
Award
The contractor must submit a certificate of insurance with the following limits:
1. Standard Worker's Compensation and Employer Liability, as required by State
Statute including occupational disease, covering all employees at the work site.
2. General Liability (Public Liability & Property Damage) (Minimum);
a. Combined single limit of $1,000,000 written on an occurrence basis;
b. Any aggregate will not be less than $1 million;
C. The contractor must purchase additional insurance if claims reduce the
annual aggregate below $1,000,000;
