The following changes are hereby made to the Ag reement include the cost/time adjustment for each change
1. Provide and install polyurethane foam (PUF) per the specifications and under the direction of the Project Manager to repair
the Fruita portal (noted in Emergency Change Order #5). The Additional Item ( #11 on bid sheet) is being used to
safeguard the
portal 10 CY FUF @ $426.00/CY for a total of $4,260.00), so there is no increase to the cost on the bid sheet. $0.00
2. Use 1 hour of machine time (noted on Additional Item #7 on bid sheet) to fill, crush and backfrll the outlet of the pond 36 -inch
culvert for safety reasons (noted in Emergency Change Order #5). One hour of equipment time will be converted to
L CMP
perform the work 1 hour @ $100.00 /hour , so there is no increase to the cost on the bid sheet. $0.00
3. Extend the completion date an additional 7 days (to April 13, 2009) due to inclement weather this spring (noted in Emergency
Change Order #5). There is no increase in cost to the bid sheet for the additional time. $0.00
4. Construct 2'/2 check dams (road /fill check dams) per the specification in the bid document on the lower east side of the main
drainage channel (noted in Emergency Change Order #6). Funding for these check dams will be from the remaining balance of
Additional Item #7 ($400.00 equipment rental hours) and the remaining balance of Additional Item #8 ($116.00 for additional
rock). Additional Items #7 and #8 will be converted to check dam construction; therefore, there is no increase in the total price
on the bid sheet. $0.00
Justification
Portal was not backfilled correctly by the previous mine operator, therefore, PUF is needed to reinforce the closure for safety and
environmental reasons. The outlet of the culvert (36° inch CMP pipe) remains exposed and could present a hazard; therefore,
[ crushing and filling the outlet will secure the culvert.
