Phillips Circle <br />Littleton, Colorado 80122 <br />(8) The applicant shall file with the Division five (5) reproducible copies of the complete permit <br />application with original signatures. Applicants proposing to mine Federal coal shall file the required number of <br />copies with the Office of Surface Mining. <br />RESPONSE <br />Five copies of the complete permit revision application are submitted to CMLRD and seven copies to the <br />OSM, Western Technical Center. TCC has developed a new format for this application. Rather than <br />address each performance standard separately, TCC has incorporated the technical responses to Rule 4 into <br />the narrative of the applicable section under Rule 2. Throughout the application, the appropriate Rule is set <br />forth in its entirety, underlined, and followed by the response of TCC to the requirements of the Rules. <br />(9) A responsible official of the applicant shall certify that the information contained in the ap lication <br />is true and correct to the best of the official's information and belief. <br />RESPONSE <br />The certification that the information contained in this application is true and correct to the best of current <br />is knowledge, information, and belief is set forth on the last page of the "Application Form for a Permit to <br />Conduct Coal Mining in Colorado' , and provided with the appropriate signatures. <br />Rule 2.03.4 - Identification of Interests <br />Each application shall contain the following information, except that the submission of a social <br />security number is voluntary. <br />(1) Each application shall contain a statement of whether the application is a corporation, partnership, <br />single proprietorship, association, or other business entity. <br />RESPONSE <br />Twentymile Coal, LLC is a business incorporated in the state of Delaware and qualified to do business in <br />the states of Colorado and Delaware. <br />MR 10 -241 2.03 -11.2 03/05/10 <br />
