(Page 3) <br />MINE ID # OR PROSPECTING ID #: M-1980-009 <br />INSPECTION DATE: August 16, 2007 <br />INSPECTOR=S INITIALS: DR? <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Problem: The existing Mining Plan is not consistent with the mining plan on file with the Division. 6 /16 / 10 <br />Corrective action: To be in compliance with Rule 6.4.3 and 6.4.4 of the Construction Materials Rules <br />pertaining to the Mining Plan and Mining Plan Maps, the Operator must submit an Amendment to <br />the Permit that contains an amended mining plan and mining plan map with sufficient detail to <br />applications or revisions must be included in sufficient detail. The operator should give particular <br />attention to: 1) Proposed locations and directions of mining from present day until the termination of <br />mining, 2) Locations and descriptions of existing and future stockpiles of product, overburden, and <br />topsoil, 3) Descriptions and locations of all processing equipment, such as screens, conveyors, <br />crushers, etc., 4) Locations and descriptions of man-made structures associated with the retail sales, 5) <br />Any water management plans or structures, including locations of pit sumps, water disposal sites, <br />storm drainage facilities, settling ponds or other impoundments, and anticipated dewatering pumping <br />rates. Operator has 90 days from the date of this inspection report, which will be 16 Jun 2010, to <br />comply with this Corrective Action. <br />PB or PV: _ B <br />TOPIC(S): AR <br />MP
