rrnni g <br />Page No. 4 <br />Permit No. COG-070000 <br />PART I <br />A. COVERAGE UNDER THIS PERMIT <br />1. Eligibility <br />In order to be eligible for authorization to discharge under the terms and conditions of this permit, the <br />owner and/or operator of any construction dewatering operation, the water being groundwater or <br />groundwater mixed with stormwater, that discharges to waters of the State, which can meet the conditions <br />identified at Part I.A.3., below, must submit a complete permit application form obtained from the' Water <br />Quality Control Division ("Division") If application applies such application shall be submitted at least <br />thirty (30) days prior to the anticipated date of first discharge to: <br />Colorado Department of Public Health, and Environment <br />Water Quality Control Division, WQCD4-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />Attention: Industrial Permits Unit <br />The application form can, be obtained from the Division or from the website at <br />summary <br />or by calling 303-692-3500. <br />Part I.A.2. <br />The Division shall have up to thirty (30) days after receipt of the application to request additional data <br />and/or deny the authorization for any articular discharge. Upon receipt of additional information the <br />Division shall have an additional 30 days to issue or deny authorization for any particular discharge. <br />If the Division determines that the operation does not fall under the authority of the general permit, then the <br />information received will be treated as an individual permit application. <br />Authorization to discharge under this general permit shall commence immediately and shall expire on <br />November 30, 2011. The Division must evaluate this general permit at least once every five (5) years and <br />must also recertify the applicant's auth ority to discharge under the general permit at such time. Therefore, a <br />permittee desiring continued coverage under this general permit must re-apply by May 1, 2011. The <br />Division will determine if the applicant is eligible to continue to operate under the terms of the general <br />permit. An application for an individual permit will be required for any point source discharge not <br />reauthorized to discharge under the reissued general permit. <br />2. Application Requirements <br />The application referenced in Part I.A.1., above, will require the following information: <br />a. The name, address, and descriptive location of the operation along with an accompanying USGS map, <br />or a map of similar quality, which shows the location of all pertinent activities and the site boundary; <br />b. The name of the principal in charge of operation, address, and phone number of the owner and of the <br />field superintendent in responsible charge; <br />c. A detailed site map that identifies all discharge points, and a schematic diagram showing the general <br />area and/or routing of the activity; <br />d. The name of water(s) receivin the discharge(s) and a listing of any downstream waters into which the <br />receiving stream flows within Ive miles of the point of discharge; <br />e. Description of the type of activity resulting in the discharge including the anticipated duration of <br />activity and/or the discharge, anticipated volume, and rate of discharge, and the source of water which <br />is to be discharged; <br />f. Description of any wastewater treatment system and recycle/reuse utilized; <br />g. A description of the methods and equipment to be used to measure flows and to analyze for pollutants <br />of concem in the discharge; and, <br />h. Storage of petroleum or chemicals on site. <br />Discharge cannot be shown to be capable of causing new or increased loadings of parameters cited in <br />Colorado's 303(d) list <br />waters; <br />3. Certification Requirements <br />for impairment for <br />The applicant must certify that the following conditions exist at the operation or the operation will not be <br />allowed to discharge under the authority of the general permit: <br />a. This permit does not constitute authorization under 33 U.S.C. 1344 (Section 404 of the Clean Water <br />Act) of any stream dredging or filling operations; <br />Revised 2/22M10
