amen <br />Earth 8 Environmental <br />approximate total of 1,750 feet was drilled. An AMEC representative observed the confirmatory <br />drilling constantly. These findings were discussed with John Lupo, P.E. Project Certifying <br />Engineer, any further remediation of this working will be done at a later date. <br />B) Mine Shaft/Stope Remediation <br />Grouting of miss (no void) holes was performed at Underground Workings #3012 and #3013. <br />C) Lateral/Slot Remediation <br />No work was performed during the week. <br />IV) Storm Water Management Structures <br />A) Control Structures <br />No work was performed during the week. <br />B) Best Management Practices <br />Best Management Practices are being observed throughout the Phase 5 VLF Project. Storm water <br />controls were maintained throughout the week. Precipitation was not substantial throughout the <br />week. <br />Storm water controls are being constructed in the East Cresson Clay Borrow as needed. <br />V) VLF Primary Underdrain <br />Construction of the Primary Underdrain continued using a CAT 330 excavator and a CAT D-9 dozer <br />to excavate the underdrain trench. Subgrade was inspected by an AMEC representative and found to <br />be competent and acceptable for geotextile placement. Geotextile (12oz/yd ) was then deployed and 6" <br />CPe perforated piping was then placed and coupled in the trench. Underdrain Fill material (UFM) was <br />then placed in a 3-foot lift in the trench using a CAT 980 front-end loader and the geotextile was <br />wrapped around the UFM, overlapping a minimum of 2 feet. Select structural fill was then placed in a <br />minimum 6" lift using a CAT 980 front-end loader and moisture conditioned (when necessary). <br />Compaction was performed using a static smooth drum compactor. Nuclear density testing was <br />performed on the placed material with passing results. Moisture-density relations were performed in <br />accordance with ASTM D-698-07. The remainder of the trench was backfilled using a structural fill <br />excavated from the trench previously. Approximately 1,050 linear feet of the Primary Underdrain was <br />installed during the week. <br />VI)VLF Geosynthetics <br />No work was performed on site during the week. <br />An AMEC subcontractor performed CQA work on manufactured liner at the Polyflex facility in Grand <br />Prairie, Texas. Samples were obtained and sent to a third party laboratory for testing, results are <br />pending. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />VLF Weekly Report WE 02.13.10.D.doc
