Stormwater Management Plan <br />Last Chance Mine <br />1. Description of Mining Activities <br />The Last Chance Mine is located on Davis Mesa in Montrose County, Colorado on claims owned <br />by Nuvemco, LLC (Nuvemco) in Township 46 North, Range 18 West, Section 12 (N.M.P.M.) as <br />shown on the Facility Map following. Elevations at the mine site range from about 6200 to 6300 <br />feet above sea level. The mine site is located by following Colorado State Highway 141 2.4 <br />miles from Naturita to Highway 90, proceeding 8.5 miles west along Highway 90, turning left on <br />to Montrose County Road DD19 Road which is followed for 3.7 miles. A slight right onto DD16 <br />Road is made and this road is followed for 3.1 miles to the existing road on right side of DD16. <br />This mining road leads to the Last Chance Mine. <br />Nuvemco is the current owner and operator of the Last Chance Mine. The mine is located on the <br />Last Chance #4 Claim. It is currently operated in accordance with the Colorado Division of <br />Reclamation, Mining and Safety (DRMS) Notice of Intent (NOI) to Conduct Prospecting <br />Operations for Hard Rock/Metal Mines No. P-2007-018. When acquired by Nuvemco, the site <br />had been reclaimed and released. Nuvemco permitted this mine site with the DRMS NOI No. P- <br />2007-018 and BLM COC-71183. Nuvemco proceeded to reopen the Last Chance Mine on a total <br />site area of 4.90 acres. The disturbed area of about 3.5 acres has facilities including a gated <br />portal, waste rock dump, and ore pile with berms. <br />Actions under the prospecting permit include a small underground uranium/vanadium operation <br />with two to four mine workers. Mining is by random room and pillar techniques and <br />conventional explosives. Equipment used in the mine include jackhammer and jackleg drills <br />using compressed air, rubber-tired skid steer loader, and a 5-ton buggy haulage unit (ore bucket). <br />A trailer mounted diesel compressor supplies air for the drills and is parked near the mine portal. <br />Water is hauled from Naturita to the site using a pick-up truck with a 350 gallon tank. The <br />ventilation fan is powered by electricity from a power drop. Ore and waste rock are mucked and <br />removed from the mine by the ore buggy which was designed and approved for underground use. <br />Ore is stockpiled on a cleared area with surrounding berms to be loaded into contracted ore trucks <br />with a loader or backhoe and hauled offsite for processing. <br />2. Area Subject to Effluent Limitations Guidelines <br />The Last Chance Mine is not covered under a CDPS industrial discharge permit because the mine <br />has no associated mine water, processing water, draining portal or adit, or runoff from any active <br />mining areas. Thus, no portions of the mine site contribute runoff to areas subject to effluent <br />limitations guidelines contained in 40 CFR subchapter N. <br />3.
