SWMP REVIEW AND/OR CHANGES <br />The SWMP will be amended whenever there is a change in design, construction, operation, or <br />maintenance which has a significant effect on the potential for the discharge of pollutants to the <br />waters of the United States, or if the control measures in the current SWMP have proved <br />inadequate. <br />10. SWMP AVAILABILITY <br />The SWMP will be provided to the Water Quality Control Division of the Colorado Department of <br />Public Health and Environment, the Colorado Division Reclamation, Mining, and Safety <br />(CDRMS), and/or the Environmental Protection Agency (EPA) upon request, and within the <br />timeframe requested. <br />11. SWMP DEADLINES AND CERTIFICATION <br />This SWMP has been prepared prior to applying for coverage under the general permit, and <br />certification of the plan development submitted with the application. <br />I certify that the Stormwater Management Plan has been prepared prior to applying for coverage <br />under the general permit. <br />Print Name Signature Date <br />This SWMP will be implemented when the facility begins operations, or whenever the general <br />permit certification is issued, whichever is later; and will be updated as appropriate. <br />"I certify under penalty of law that a complete Stormwater Management Plan, in compliance with <br />Part I.C. of the permit, has been prepared and implemented for my facility. Based on my inquiry <br />of the person or persons who manage the system, or those persons directly responsible for <br />gathering the information, the Stormwater Management Plan is, to the best of my knowledge <br />and belief, true, accurate, and complete, and implemented as written.
