Cattany <br />Division Director <br />Natural Resource Trustee <br />The Colorado Division of Reclamation, Mining, and Safety (CDRMS) has reviewed and found the <br />above referenced report to be in compliance with the following CDRMS regulations: <br />4.09.1(11)(a) Inspected at least quarterly by a reregistered engineer or other qualified professional <br />specialist in construction of earth and rockfill embankments under the direction of a <br />qualified engineer throughout the construction <br />The certified report covers the fourth quarter monitoring period from September to <br />December 2009 and the inspections were conducted by a certified engineer. <br />4.09.1(11)(b) Provide to the Division a certified report within 2 weeks after each inspection that the <br />fill has been constructed as specified in the design approved by the Division, The <br />report shall include a description of any appearances of instability, structural <br />weakness, and other hazardous conditions. <br />The inspection was conducted on December 30, 2009 and the report was received by <br />the Division on January 11, 2009. No instability, structural weakness, or other <br />hazardous conditions were noted during the December 30 inspection. <br />4.09. ](1]) (c) Any certified report addressing the underdrain system and protective filters shall <br />include color photographs taken during and after construction, but before underdrain <br />are covered with excess spoil. If the underdrain system is constructed in phases, each <br />phase shall be certified separately. ...photographs shall be taken of the underdrain as <br />the underdrain system is being formed.
