October 2004 <br />-28- <br />043 -2170 <br />6.0 GEOSYNTAETIC CQA TESTING ANT) MONITORING <br />• <br />• <br />The following sections describe the CQA activities performed by Golder for the geomembrane <br />installed in the Phase We OSA, and the geosynthetics installed in the LDT and Underdrains. <br />In accordance with the Golder CQA Plan, when a Golder monitor observed deficiencies, the monitor <br />determined the nature and extent of the problem and notified Poly -Flex and/or CC &V. When <br />unsatisfactory test results identified a deficiency, Golder performed additional tests to define the <br />extent of the deficient area. Poly -Flex corrected deficiencies to meet the requirements of the <br />specifications, and Golder retested and /or approved the corrected deficiency prior to any additional <br />related work. <br />6.1 Ore Storage Area Geomembrane <br />CQA activities conducted by Golder during the installation of the geomembrane in the Phase IVe <br />OSA consisted of the following: <br />• Conformance sampling and testing of the geomembrane; <br />• Observation and approval of the SLF surface prior to deployment of 80 -mil <br />single-sided textured LLDPE geomembrane in the OSA; <br />• Observation and documentation of geomembrane deployment, including verifying <br />sheet thickness and overlap and documenting panel layout; <br />• Observation of trial seam sample testing and documentation of results; <br />• Observation and documentation of field seaming; <br />• Identification of destructive seam sample test locations, observation of destructive <br />seam sample testing, and documentation of test results; <br />• Observation and documentation of geomembrane repairs; and <br />• Observation and documentation of non - destructive pressure testing of fusion seams <br />and vacuum testing of extrusion seams and repairs. <br />The following sections present a detailed description of each of the above activities. <br />L• 1011217010 WCcrt- QAph1Vo-10WIN32IMW00.WBSCeftn[VLd c <br />Golder Associates <br />
