Cripple Creek & Victor Gold Mining Company <br />Cresson Project ��� <br />Quality Assurance Monitoring and Test Results <br />Liner Recertification <br />December 17, 2009 <br />5.0 GEOSYNTHETICS CQC SUBMITTALS <br />Geosynthetics CQC submittals reviewed by AMEC included installation personnel resumes, <br />geomembrane resin certificates, geomembrane roll certificates, and welding rod certificates. <br />AMEC's inventory control, summarizing CQC certification received for each roll of <br />geomembrane received, is presented in Appendix H. All geosynthetic material used met project <br />specifications. <br />5.1 Geomembrane Installation Personnel Resumes <br />AMEC reviewed the resumes of Lonestar installation personnel to verify that the installation <br />superintendent and the seaming personnel possessed sufficient experience as required by the <br />project specifications. Review of the installation personnel resumes indicated that key <br />personnel had experience sufficient to meet or exceed project specifications. Geomembrane <br />personnel resumes are presented in Appendix H -2. <br />5.2 Geomembrane CQC Documents <br />CQC documents submitted by Poly -Flex included resin supplier certificates and geomembrane <br />manufacturer resin certificates, geomembrane roll certificates, and welding rod certificates. <br />AMEC reviewed CQC documents prior to geomembrane placement and verified that the <br />documentation was complete and that the material properties and CQC testing frequency <br />required by specifications were met. <br />5.2.1 Resin COC Certificates <br />LLDPE polymer raw material (resin) CQC certificates were provided by the resin manufacturer, <br />Chevron Phillips Chemical Company, at a frequency of one per rail car shipment. AMEC <br />reviewed these certificates prior to geomembrane placement. The geomembrane manufacturer <br />also performed density and melt index tests of the resin supplied by Chevron Phillips Chemical <br />Company to verify material properties prior to geomembrane manufacture. The CQC <br />certificates provided by the resin manufacturer are presented in Appendix H -3. <br />5.2.2 Roll COC Certificates <br />Poly -Flex provided CQC certificates for each roll (approximately every 6900 ft of LLDPE <br />geomembrane used for the project, exceeding the required minimum frequency of one per <br />50,000 square feet of geomembrane. AMEC reviewed the CQC certificates to verify <br />conformance of the geomembrane to the project requirements.
