Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Liner Recertification <br />December 17, 2009 <br />amec� <br />Phase IVa and Phase IVc LOB: In the recertification areas, AMEC measured the <br />thickness, density, moisture content, and permeability of the SLF placed during the <br />Phase IV activities. <br />AMEC visually monitored the placement of Drain Cover Fill (DCF) within recertification <br />areas, including verification of lift thickness and grade control, identification and removal <br />of deleterious materials, and verification that care was exercised in fill placement to <br />avoid geomembrane damage. AMEC performed CQA laboratory testing of grain size <br />and Atterberg limits. <br />AMEC monitored the ambient air temperatures and the temperatures of all fills, including <br />SLF, in accordance with project specifications. <br />1.2.2 COA/CQC Monitoring of Geosynthetics Installation <br />AMEC provided CQA monitoring of geosynthetics used for the recerification of the Phase IVa <br />ramp and Phase IVc LOB. CQC services provided by AMEC consisted of performing <br />geosynthetics inventory and checking quality control certifications prior to geosynthetics <br />deployment. AMEC performed geosynthetics CQA activities in accordance with the CQA Plan. <br />0 Geosynthetics CQA activities performed by AMEC included: <br />■ Observation and documentation of geomembrane deployment, trial seams, seaming, <br />destructive testing, and non - destructive testing (pressure tests and vacuum tests); <br />■ Performing conformance testing of geomembrane; <br />■ Measuring geomembrane panel thickness and confirming adequate panel overlap; <br />■ Visually inspecting geomembrane sheet quality, sheet grinding, seam alignment, and <br />continuity; and <br />■ Retaining archive samples of geomembrane destructive tests and trial seams. <br />1.3 Project Coordination <br />AMEC CQC and CQA activities were directed by the AMEC Resident Engineer. The AMEC <br />Resident Engineer for the period was Mr. Joseph D. Hickey. The Certifying Engineer, <br />Mr. John F.
