Cripple Creek & Victor Gold Mining Company Cresson Project am� Quality Assurance Monitoring and Test Results Liner Recertification December 17, 2009 1.2 Construction Quality Assurance /Quality Control AMEC provided CQA monitoring and testing services, and prepared this report describing the monitoring and test results for the recertification of these areas. Figures and supporting CQA monitoring and testing documentation are presented in appendices attached to this report. Since the recertification areas are located adjacent and /or downgradient of previously certified areas, sections of the Phase IVa and Phase IVc certification reports relevant to the recertification of the noted areas are provided in this report to provide continuity in the areas being certified (see Appendix A). 1.2.1 CQA Monitoring of Earthworks AMEC provided CQA testing and monitoring of earthworks for the Phase IVc LOB and Phase IVa ramp recertification. CQA testing and monitoring of earthworks was provided for in -place materials in accordance with the AMEC Earthworks CQA Plan. The earthworks material specifications for this project are summarized in Table 1. CQC earthworks testing was provided to support the contractor during the manufacture of materials (DCF and SLF) and prior to material placement. AMEC provided CQA laboratory and field testing, and monitored earthworks construction activities as follows: Phase IVa: AMEC monitored site grading, including slope re- contouring and fill placement, lift thickness, moisture conditioning, temperatures, and compactive effort. and use of suitable material for SF. Phase IVa: AMEC visually monitored subgrade preparation to confirm that the subgrade surface was unyielding, free of organic matter and other unsuitable materials, and that the subgrade surface was compacted prior to Soil Liner Fill (SLF) placement. Phase IVa: AMEC visually monitored the placement and compaction of SLF including observation of depth verification performed by Ames, particle size, moisture conditioning, and compaction of the fill. AMEC performed in -place density and moisture content testing, and performed CQA laboratory testing of grain size, moisture content, Atterberg limits, compaction, and permeability. AMEC also provided visual monitoring of the SLF surface preparation and moisture conditioning for geomembrane deployment, including verification that the surface was proof - rolled, compacted, and hand - worked so as to be free of desiccation cracks, abrupt grade changes, and sharp stones or objects.
