Cripple Creek & Victor Gold Mining Company <br />Cresson Project am� <br />Quality Assurance Monitoring and Test Results <br />Liner Recertification <br />December 17, 2009 <br />1.2 Construction Quality Assurance /Quality Control <br />AMEC provided CQA monitoring and testing services, and prepared this report describing the <br />monitoring and test results for the recertification of these areas. Figures and supporting CQA <br />monitoring and testing documentation are presented in appendices attached to this report. <br />Since the recertification areas are located adjacent and /or downgradient of previously certified <br />areas, sections of the Phase IVa and Phase IVc certification reports relevant to the <br />recertification of the noted areas are provided in this report to provide continuity in the areas <br />being certified (see Appendix A). <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQA testing and monitoring of earthworks for the Phase IVc LOB and Phase <br />IVa ramp recertification. CQA testing and monitoring of earthworks was provided for in -place <br />materials in accordance with the AMEC Earthworks CQA Plan. The earthworks material <br />specifications for this project are summarized in Table 1. CQC earthworks testing was provided <br />to support the contractor during the manufacture of materials (DCF and SLF) and prior to <br />material placement. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks construction <br />activities as follows: <br />Phase IVa: AMEC monitored site grading, including slope re- contouring and fill <br />placement, lift thickness, moisture conditioning, temperatures, and compactive effort. <br />and use of suitable material for SF. <br />Phase IVa: AMEC visually monitored subgrade preparation to confirm that the subgrade <br />surface was unyielding, free of organic matter and other unsuitable materials, and that <br />the subgrade surface was compacted prior to Soil Liner Fill (SLF) placement. <br />Phase IVa: AMEC visually monitored the placement and compaction of SLF including <br />observation of depth verification performed by Ames, particle size, moisture conditioning, <br />and compaction of the fill. AMEC performed in -place density and moisture content <br />testing, and performed CQA laboratory testing of grain size, moisture content, Atterberg <br />limits, compaction, and permeability. AMEC also provided visual monitoring of the SLF <br />surface preparation and moisture conditioning for geomembrane deployment, including <br />verification that the surface was proof - rolled, compacted, and hand - worked so as to be <br />free of desiccation cracks, abrupt grade changes, and sharp stones or objects.
