6.4.1 Exhibit A - Legal Description <br />3. Rule 6.4.1(1) requires the applicant to locate the main entrance to the mine site using <br />coordinates of latitude and longitude in degrees, minutes and seconds or in decimal degrees <br />to an accuracy of at least five (5) decimal degrees. Please provide the coordinates of the main <br />entrance to the mine site. <br />6.4.2 Exhibit B - Index Map <br />No comment. <br />6.4.3 Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />4. The applicant submitted a single map for the Mining Plan and the Reclamation Plan. Please <br />submit a separate Mining Plan Map and Reclamation Plan Map. <br />5. Rule 6.4.3(b) requires the applicant to list the names of all power and communication lines <br />on the area of the affected land and within 200 feet of all boundaries of such area. During the <br />inspection conducted on October 15, 2009 an overhead communication line was identified on <br />the west side of the affected land. Please add this structure to the Mining Plan Map. <br />6. In accordance with Rule 6.4.3(e), please specify the type of present vegetation covering the <br />affected lands on the Mining Plan Map. <br />7. The Mining Plan Map was not signed by a registered land surveyor, professional engineer, or <br />other qualified person. Please submit a signed copy of the Mining Plan Map as required by <br />Rule 6.2.1(2)(b). <br />Rule 6.4.3(g) requires the applicant to show the owner's name, type of structures, and <br />location of all significant, valuable, and permanent man-made structures contained on the <br />area of the affected land and within two hundred (200) feet of the affected land. Please <br />include all permanent man-made structures on and within 200 feet of the affected land, <br />including: fences, scalehouses, outbuildings, water wells, etc. <br />9. Please locate topsoil stockpiles, overburden stockpiles, and product stockpiles on the Mining <br />Plan Map. Also, show the location of the portable crusher. <br />6.4.4 Exhibit D - Mining Plan <br />10. The applicant stated in Exhibit D the newly expanded pit area will be mined `over a 10 over a <br />5 year period.' Please clarify the approximate length of the mining operation. <br />11. In accordance with Rule 6.4.4(e)(ii), please provide a description of the size and location of <br />each area to be worked during each phase.
