12. The Mining Plan states the pit will be excavated to groundwater. However, it is also stated <br />that the pit will be mined to approximately 1 foot above the groundwater, depth. The exposure <br />of groundwater requires a well permit and substitute water supply plan(SWSP) from the <br />Office of the State Engineer. Please commit to either obtaining a well permit and SWSP or <br />remaining 2 feet above the groundwater level. <br />13. The applicant has stated an earthen berm will be used to control stormwater runoff. Please <br />indicate the location of the earthen berm on the Mining Plan Map. Also, specify the <br />dimensions of the earthen berm. <br />14. Rule 6.3.3(1) requires the applicant to describe what measures will be taken to minimize <br />disturbance to the hydrologic balance. Please describe what will happen to the drainage swale <br />during mining operations. Will the swale be relocated or routed around the pit? Will <br />stormwater be routed into the pit? <br />6.3.4 Exhibit D - Reclamation Plan <br />15. The Reclamation Plan states the drainage swale will be redirected across the site. Please <br />specify the depth and width of the redirected drainage swale. Also, please clarify if topsoil <br />will be placed over the swale. <br />16. The Division will calculate the reclamation costs for this site once all of the adequacy issues <br />have been addressed. <br />6.3.5 Exhibit E - Map <br />17. The Mining Plan Map was not signed as required by Rule 6.2.1(2). Please submit a signed <br />copy of the Mining Plan Map. <br />18. As required by Rule 6.3.5(2)(b), please label the names of the owners of record of the surface <br />of the land within 200 feet of the affected area on the Mining Plan Map. In addition, identify <br />the type of structure and owners of any permanent or man-made structures within 200 feet of <br />the affected land. <br />19. The Mining Plan Map shows the western permit boundary is immediately adjacent to County <br />Road 189. Please specify the extent of the County right-of-way on the west side of County <br />Road 189. <br />20. The Reclamation Plan Map states the pit floor will slope to the west and south to retain <br />stormwater runoff. Please explain why the pit floor will be sloped to contain stormwater, <br />instead of directing stormwater runoff to the drainage swale so that it may flow to the Big <br />Sandy Creek.
