5. Please describe the proposed surface excavations and disturbances including adits, <br />roads, processing areas, water diversion areas, ponds, buildings, pits, trenches, <br />waste piles, collar areas of underground workings, etc. in detail. <br />6. Please describe the proposed area of the above disturbances in detail and include <br />any other disturbances not listed elsewhere. Will there be any chemicals and fuels <br />stored on site? If so please list types and quantities. <br />7. Please include all roads that will be used in conducting any type of prospecting <br />activities including existing roads that will be upgraded and new roads that will be <br />constructed. <br />8. Based on our inspection, the Division knows there will be significantly more area <br />disturbed than zero as you have indicated . Please provide the total project area to <br />be involved including all areas associated with prospecting including adits, <br />buildings, roads, excavations, etc. <br />9. During the inspection several pieces of equipment were observed and it was <br />indicated that they were being used to conduct prospecting including a small <br />trommel, a front end loader, water pumps, water tanks, buildings, etc. Please <br />describe all equipment to be used in the prospecting operation. <br />10. Describe and locate any structures to be constructed and used in the prospecting <br />operation including ponds, roads, stockpiles, buildings, etc. Once again, during <br />the inspection several items were identified that should be included in this section. <br />11. Describe anticipated relationship to surface and groundwater including proximity <br />to streams, penetration of groundwater aquifers, etc. The site is very clearly <br />located on both sides of Spring Gulch and water was obviously being diverted <br />from this gulch for use in processing. Furthermore, the process water was being <br />discharged back into the gulch with very little treatment. Please refer to Rule 3.1.6 <br />and 3.1.7 for specific requirements concerning protection of surface and <br />groundwater. This will aid you in addressing this section. <br />Section IV-Operation and Reclamation Measures <br />Please address each subsection in detail including: <br />1. A photographic record of pre and post-disturbance activity is suggested. <br />2. Please provide a description of the native vegetation of the area to be disturbed. <br />3. Based on the inspection, there appears to be more than 2-3 inches of topsoil <br />available for reclamation. Please re-evaluate and revise this section. In addition <br />to the topsoil depth, please describe how topsoil will be salvaged, stockpiled and <br />re-distributed for vegetative re-establishment including the location of topsoil <br />stockpiles. <br />4. Not applicable if no drill holes are anticipated. <br />5. Describe how portals, adits, shafts, ponds, excavations and other disturbances will <br />be reclaimed. <br />6. Describe how roads will be reclaimed. <br />7. List the seed mixture to be used in re-establishment of vegetation and describe the <br />method for seed bed preparation and the application method for seeding (ie-drill <br />seeding, broadcast seeding, transplant seedlings, etc.) "None needed" is not an <br />acceptable response.
